The ESI Scheme in the North Eastern States was implemented as far back as in 1958, under the Regional Office, West Bengal. It was in the early 70s that an independent Region was created for North East Region, with its Regional Office at Guwahati. Initially, the Scheme was implemented in 5 (five) Centers in the state of Assam. Gradually, with the continuous and optimum efforts, the Scheme had been extended to the state of Assam and to the state of Meghalaya in the year 1980. Thereafter, the Scheme was extended to the state of Nagaland in the year 2008, Tripura in the year 2009. Mizoram in the year 2015, Manipur in the year 2018 and Arunachal Pradesh in the year 2020.
There are 122 nos. of districts in North East Region out of which ESI has been implemented in 51 districts (Partial / fully notified). The state wise figure of number of implemented districts are as below.

At present there are 20 numbers of Branch Offices and 03 DCBOs (Dispensary cum Branch Office) across North East Region (NER) to facilitate the beneficiaries and disbursement of Cash Benefits. And to provide primary health care, there are 37 numbers of dispensaries and 28 numbers of Insurance medical practitioners (IMPs) engaged by State Govt. There is 01 ( 50 bedded) ESIC Model Hospital at Guwahati. There are also 29 tie up hospitals & Diagnostic Centres (Assam – 24, Manipur – 02, Mizoram – 02 & Tripura - 11) engaged by State Govt. for providing secondary medical care and diagnostic facilities
Recently, State Govt. run ESI Annexe Hospital, Tinsukia is taken over by Corporation and construction of 100 beded ESIC Hospital in Tinsukia is under process.
